Patient’s Rights, Responsibilities and Privacy Nayak Plastic Surgery

Your Rights as a Patient:

You have the right to be treated with respect, consideration and dignity.

You have the right to have all disclosures and records treated confidentially. You have the right to approve or refuse the release of records, except when prohibited by law.

You have the right to participate in decisions involving your healthcare, except when such participation is contraindicated for medical reasons

You have the right to be informed to the degree known, complete information concerning their diagnosis, evaluation, treatment and prognosis. If medically inadvisable to discuss with the patient, this information will be provided to the person designated by the patient or the legally authorized person.

You have the right to be informed of all reasonable options or alternatives for care and/or treatment and of the potential advantages and disadvantages of each of the alternatives to having a procedure performed in an office or other out-patient facility.

You have the right to copies of your medical records at a nominal cost and, if you request it, those records will be transferred to another practioner in a timely manner.

You have the right to refuse any diagnostic procedure or treatment and to be advised of the likely medical consequences of such refusal.

You have the right to know the conduct expected of you in the facility and the consequences of failure to comply with these expectations.

You have the right to know services available at this facility.

You have the right to know provisions for after-hours emergency care.

You have the right to know if any of the planned procedures or treatments is a part of a research study and the right to refuse participation in that study.

You have the right to know whether or not your providers are insured.

You have the right to change providers.

You have the right to know how to go about expressing suggestions to the facility and the policies regarding grievance procedures and external appeals in the event that you were dissatisfied with your treatment.

You have the right to know what fees are expected and what payment policies are.

You have the right to additional information about and/or implementation of advance directives for healthcare, as described in the clinic policy and procedure.

You have the right to all of your rights outlined above.

Your responsibilities as a patient:

You have the responsibility to provide complete and accurate information to the best of your ability about your health, any medications, including over-the-counter products and dietary supplements and any allergies or sensitivities.

You have the responsibilities to notify your practioner of any changes to your health including but not limited to: new medications, diagnosis, or procedures during the course of your treatment both prior to and following your procedure.

You have the responsibility to provide a responsible, steady adult to transport you home and remain with you for a minimum of 24 hours if required by the provider.

You have the responsibility to inform your provider with the information about the existence of a living will or advanced directive related to your health care as well as medical power of attorney.

You have the responsibility to accept any financial responsibility as promptly as possible.

You have the responsibility to be respectful of all health care providers and staff, as well as other patients.

You have the responsibility to report perceived risks in care and changes in your conditions or symptoms.

You have the responsibility to ask questions when you do not understand the recommended care or treatment and what the patient is expected to do.

You have the responsibility to keep appointments promptly and notify the practice with reasonable advance notice when you are unable to do so.

If you have any concerns or questions, please ask. Staff will not be aware of your concerns unless you share them. Having questions answered will help you understand your care and will help us continue to improve.
