In contrast to a traditional facelift in which the skin is pulled tight over the face’s aging underlying structure, a deep plane extended facelift adjusts the deeper tissues in the face for more comprehensive and natural-looking results.

While traditional and SMAS (superficial muscular aponeurotic system) facelifts place increased tension on the facial skin and muscles, a deep plane extended facelift fully releases, and passively repositions the underlying tissues of the face to restore their youthful positions without imposing additional strain on the skin or muscles. This approach allows for powerful results without the overly stretched, “done” appearance of traditional or SMAS facelifts.

Advanced anesthetic techniques and monitoring allow for comfortable, restful sleep, with no awareness or recollection of surgery and no need for a breathing tube or ventilator - all while keeping the total anesthetic administered to an absolute minimum. We regret that patients whose baseline health is borderline (BMI over 30, smokers, heart/lung/kidney disease, etc) may not be candidates for surgery.

Before your procedure

From your initial consultation through your recovery, there are several steps that you will need to take to ensure that your treatment goes as smoothly as possible. At your consultation with Dr. Nayak, you will have the opportunity to express in detail all of your specific concerns, your medical history, and any medications you may be taking.

Dr. Nayak will also perform a thorough physical examination to identify what corrections will yield the best results. Once you and Dr. Nayak have established that a deep plane facelift is the best treatment option for you, we will schedule your pre-op and surgical appointments. Your pre-op appointment will take place two weeks before your surgery. At this appointment, you will receive instructions for before and after your procedure and all of your prescriptions so you can fill them before surgery.

With any surgical procedure, it is necessary to avoid any medications or supplements that may thin the blood two weeks before and two weeks after your surgery. Dr. Nayak or one of his associates will take your pre-op photos at this appointment as well. You will also fill out your surgical consent form at this time, and your payment will be due. By taking the necessary steps to prepare for your procedure, you can arrive for surgery assured that everything you will need is already in place.

The deep plane facelift procedure

The night before your surgery, you should not eat or drink anything after midnight. If Dr. Nayak has instructed you to take any medications (including antibiotics, oral sedatives, or blood pressure medication) on the morning of your surgery, you should take them with a small sip of water. Do not take any diabetes or ADHD medication on the morning of your surgery.

Once you have arrived, you will once again go over your medical history and surgical plan. Next, Dr. Nayak’s team will prepare you for surgery by placing various monitors and air-filled socks to aid circulation. Once you are ready, intravenous sedation and local anesthesia will be administered, and surgery will begin.

Through small, strategically placed incisions, Dr. Nayak will reposition the collective tissues on the face and secure them in a younger, prettier shape before meticulously closing your incisions. Your incisions will be very well-hidden, tucked beneath the jaw and around the ears. Once completely healed, your incisions will be nearly invisible.

You must have a close friend or family member drive you to and from your surgery, as well as a capable adult to stay with you for the first 24 hours. In most cases, we will ask you to arrive at our office at 7:15 in the morning. Deep plane extended facelift surgery typically takes about four hours, although your surgery may take longer if other procedures are added. Your companion will be contacted by telephone when your surgery is complete and you are in recovery.

Deep Plane Facelift Recovery

Because a deep plane extended facelift occurs at the natural plane within the face and avoids the separation of connected tissues, swelling and bruising are minimal compared to the effects of a non-deep plane extended facelift. Dr. Nayak may prescribe pain medications to reduce discomfort during your recovery, and cold compresses can also help to alleviate swelling.

You will need to stay within 30 minutes’ drive of the office the first night or two. Dr. Nayak’s team can suggest one of our prescreened hotel options if you like. You will see Dr. Nayak and his aftercare team the day after surgery in addition to three to five other visits over the first 2 weeks. Traveling patients must commit to spending the surgery day plus seven additional days in St. Louis, or within easy driving distance, before flying home. Most patients return to their normal social activities around three weeks.

Many patients go ten to fifteen years before follow-up surgical treatment is needed.
